Clinical Pharmacy Specialist - BU

As a Clinical Pharmacy Specialist at the John J.
Pershing VAMC, Poplar Bluff, MO, you will specialize in planning, implementing and operating drug monitoring and Pharmaceutical Drug Management.
Duties include but are not limited to:
o Serves as a mid-level provider that can initiate, modify, or discontinue medication therapy in accordance with approved prescriptive authority;o Assess the appropriateness and effectiveness of the patients' medication regimen by reviewing medical records or interviewing patients as appropriate; o Recommend additions or deletions in the medication regimen;o Consult appropriate specialist when conditions deem necessary;o Educate patients on proper use and care of their medications;o Order appropriate lab tests to ensure patient safety;o Participate in review of extended care, long term care, and end of life patient profiles;o Conduct Poly-Pharmacy reviews; o Monitor adverse drug reactions on a continuous basis;o Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served; o Communicates with customers/patients in a courteous tactful and helpful manner; o Provide general cognitive, physical, emotional and chronological maturation of geriatric patients; o Clinical Duties may include anticoagulation duties; ando Implements and monitors formulary management protocols and programs approved by the Pharmacy and Therapeutics Committee.
Work Schedule :
Monday - Friday, 8am to 4:
30pm with rotating late nights, weekends, on call and holiday rotation.
